Health Insurance For Children
Children in Germany are not automatically assigned to a specific health insurance system. Parents are free to choose whether to insure their child under public (GKV) or private health insurance (PKV).
The most suitable option depends on the family’s current insurance situation — an important factor is also whether the parents are married or not, as this can affect the child’s eligibility for free family co-insurance.

Advantages:
✅ Free family insurance:
If both parents are publicly insured — or the higher-earning parent is in the public system — children can be covered free of charge.
➜ If the parents are unmarried, the income of the privately insured parent does not affect the child’s eligibility for free family insurance under the public health system
❗Important:
Children are generally free to be insured under either the public (GKV) or private (PKV) system.
Disadvantages:
❌ Treatment decisions are largely determined by statutory regulations.
❌ Services are limited by the principle of cost-effectiveness (§12 SGB V).
❌ Alternative treatments and extended health checks (e.g. speech or motor skills tests outside the standard U-examination program) are usually not covered.
❌ Longer waiting times for specialist appointments.
❌ If free family insurance is not possible, the minimum contribution of approx. €260/month applies.
Premium calculation:
The premium depends on your child’s health status and the chosen plan, starting at €135 per month for a plan without deductible.
Long-term care insurance is free of charge if at least one parent has private health insurance.
Advantages:
✅ You become the direct contracting partner of the doctor, which means you can freely choose who treats your child and jointly agree on the best therapy on an equal footing.
✅ Faster access to medical appointments
✅ Flexible plan design, e.g. including alternative practitioners (Heilpraktiker treatments)
Disadvantages:
❌ A health check is required before admission — coverage is available only for healthy children.
💡 Children can be included without a health check if one parent is already privately insured (specific conditions apply).
