Civil servants in Germany can generally choose between statutory (public) and private health insurance.
Unlike employees or self-employed individuals, civil servants receive a partial reimbursement of medical expenses from their employer (Dienstherr). They only need to insure the remaining portion of their healthcare costs.
Because only private health insurance (PKV) offers this type of partial coverage (Restkostenversicherung), most civil servants opt for a private plan. It is usually the more cost-effective and needs-based solution.
If a civil servant chooses statutory health insurance (GKV) instead, they forfeit the reimbursement entitlement from their employer and must pay the full contribution themselves — unless their federal state provides what is known as a “pauschale Beihilfe” (flat-rate allowance).
